  5. Could you give an opinion as to whether a callus is considered a stage II decubitus?

  6. Is it necessary to have a LVN supervisory visit done at Discharge? I thought supervisory visits, both for LVN and aide were to be done if theses services were ongoing, so does not need to be done at discharge assessment. CMS information suggest agency policy only as the answer.
