HHQI releases best practices on patient self-management

Selman-Holman Associates is a coordinator with this, and it is exciting! HHQI now has released its Best Practice Intervention Package on patient self-management.

Are you familiar with your community’s efforts to improve the 30-day readmission rates of patients to your local facilities? Most communities around the country are addressing this issue in one form or another. Have you been contacted by someone in your community to discuss this issue? If not, you may want to ask yourself, ‘why not?’

As we all know, home health plays a central role in helping patients regain their health and remain in their homes. If you are not currently ‘at the table’ for your community discussions, now is the time to contact other providers –other home health agencies as well as hospitals, nursing homes, SNFs and physician offices – and begin the discussion as to what you can do as a community to improve the quality of care you provide to your citizens as they move from one care setting to another.

The Home Health Quality Improvement (HHQI) national campaign recently released the focused Best Practice Intervention Package (BPIP) on the topic of Patient Self-Management. This BPIP is much smaller than the previous packages updating the content published in 2011.

If you haven’t downloaded it yet, please do so and share with your community partners. To download this, please go to the HHQI website and either register or login.

The BPIPs are found under the “Education” tab on the blue tool bar.

HHQI national campaign kick-off webinar!!

The Home Health Quality Improvement (HHQI) National Campaign is ready to kick-off its third phase with a free webinar Tuesday, Sept. 18, from 4:00–5:30 pm ET. If you can’t make it to the live webinar, a recording will be posted to the HHQI website by Sept. 21.

The focus of the webinar will be HHQI website enhancements including tools, BPIPs, and resources. One new feature to the webinar is that you still need to login, but you can stay logged in indefinitely, if you wish.

Phase 3 of the Home Health Quality Improvement (HHQI) is about to start with a national campaign launch of Sept. 18.

Selman-Holman Associates is a network coordinator that will lead groups of HHAs in quality improvement movements. We’ll also keep you informed of  HHQI events

HHQI data reports are now available

The January 2011 HHQI reports for ACH and Oral Medications are now available online  These reports provide a look into the potential causative factors of ACH rates and also focus on M2020.
The reports are available in PDF, CSV, XML, MHTML, Excel, TIFF, and Word.
Here is how to get to the data:

1. Go to the HHQI Web site, www.homehealthquality.org

2. Click on the Quick Link (right side) for “HHQI Data Access System.” This will lead you to a secure website.
3. Log in with your username and password.

HHQI releases best practice intervention package

The Home Health Quality Improvement Cross Settings Best Practice Intervention Package (BPIP) series has begun. The first in a series of three dealing with best practice education, this one on cross settings, was published on October 28th and is currently available on the HHQI Web site at www.homehealthquality.org.
